Community Provider Support Tools
Children’s Health has developed a variety of resources that health care professionals can use to give to their patients and families.
Resources for parents include over 22 medical social and behavioral health topics. Many of the documents are available in English and Spanish. The medical social resource topics range from camps and activities for children, to guides on medical decision making, insurance and financial information.
The behavioral health resources include support for parents related to children with learning disabilities, ADHD, and autism to parenting advice around sleep and technology use.
Resources for your practice include many tools and information to guide the transition of a child to an adult provider, and information on the nursing care of a child dependent on technologies, such as a central venous catheter, tracheostomy, ventilator or gastrostomy tube.

Transition to Adult Care Toolkit
The process of moving from pediatric to adult care, or health care transition, is an exciting and challenging time for teens and young adults with and without medical complexities. Transition takes time, with planning, preparation, moving to an adult model of care, and eventually transferring to new adult doctors. The American Academy of Pediatrics (AAP) recommends this process begins by age 16, including legal and medical decision-making changes at age 18, with transition occurring between ages 18 and 21.
At Children’s Health, the Office of Patient Transition supports the health care transition of teens, young adults, and families across the Children’s Health Network.
This toolkit includes educational tools for teens, young adults, and families as they move through their journey to transition to adult care. These resources are for healthy teens and those with a chronic condition will be independent in the management of their health care and medically complex teens and young adults who also may have parents or caregivers involved in their care into adulthood.
