Food and Nutrition Services Student Information Fact Sheet

School/Facility/Student Responsibility

Once the school/facility has determined student assignments, the start and finish dates should be sent to Tamara Akens.  The contact should be made 3 months prior to the start of the rotation.  Once approved by the hosting department, the student will receive a Student Request form from the hosting department.  This form must be completed and returned to the hosting department as soon as possible.  The student must complete all forms, computer based training (CBTs), etc. as requested in a timely manner.  Failure to complete in a timely manner will delay rotation opportunity, and the time may not be made up.

The school/facility is responsible for sending attestation letters which include background checks and drug screens for each student.  Additionally the school/facility must send updated insurance documents and registration/ licenses as requested for instructor and or professor.

Hosting Department Responsibility

Once the Student Request Form is returned to the department it will be forwarded to Student Services.  This starts the process and Student Services will send the student a confirmation email when the student is cleared to start. 

The hosting department is responsible for making sure the student receives a department orientation and instructions needed for electronic medical records training as applicable.
