Complex Care Medical Services
Children’s Health now offers a service specifically dedicated to caring for the pediatric population with complex, chronic medical conditions. For primary care physicians in the community, time and resources may not be available. Children’s Health is here to help. Complex Care Medical Services is comprised of two main programs - Care Coordination Program and Medical Home.
Why Children's Health℠?
To achieve the triple aim of the right care, at the right place, at the right time through centralized, coordinated, comprehensive, and compassionate care with the intent to improve the child’s growth and development, the family’s satisfaction, and decrease unnecessary hospital utilization.
To optimize care for children with medical complexity through partnerships with caregivers and medical neighborhoods so that children can reach their greatest potential and experience life more fully.
