School Responsibility: Calendar start and finish dates vary per school and types of rotations.  All schools must contact Lena M. Pate, MT AHI(AMT) well before start dates for student and rotation approval.  Once reviewed and approved by hosting department, we will need the student’s email address to send them the necessary information and Student Request form to complete and return to the Laboratory. 

Student Responsibility: Once the Student Request Form is returned to the Lab, we will forward on your approved form to Student Services.  Student Services will then email the student with directions on how to go through the process for a rotation at Children’s, including requirements for a rotation, student paperwork, and any training requirements prior to being cleared to start rotation.  These must be completed at least one week (Monday-Friday) prior to start date.   Students are responsible for working with Occupational Health and Student Services to ensure they receive their clearance email at least one business day prior to start date.  Rotations will be delayed or possibly canceled if this is not completed before your scheduled start date.
