We provide care across the continuum, from the primary care office to the hospital and back to the home. Children’s Health℠ Home Care provides family-centered, comprehensive care to children with chronic illnesses, as well as those recovering from hospitalization or injury. We serve as a link between families, hospital providers and the patient’s primary care physician. Under a physician’s order and insurance authorization, Children’s Health Home Care supports a variety of conditions for children at home.

Our Home Care Division is accredited by The Joint Commission, which means that the quality of our care is periodically evaluated by quality experts to ensure that we are continuing to provide the highest level of care to our patients.

Comprehensive Care and Support

Children’s Health Home Care services provides patients and families pediatric care from skilled nurses, physical therapists, occupational therapists, speech therapists and respiratory therapists. We also offer a wide range of home medical equipment and supplied right to the home to help our most fragile patients. Our team can help arrange the purchase or rental of specialized medical equipment and supplies. Equipment will be able to be delivered to your home by our experts, who will also teach you how to use it.

Parent Resources

We know setting up home care for your child can be stressful. Who do you call? How do you get equipment? What about diet or pain management? What are the safety and security measures? The Children’s Health Home Care Welcome Packet  can help answer your questions and concerns before your child transitions home.Get general information like scheduling, safety and hours. Learn about patient care including admission, infection prevention, discharge and more. You can also learn about our specific services and home medical equipment (HME) items available.
