At Children’s, we believe improving the health of children in our community begins with establishing a high-quality and affordable patient-centered medical home (PCMH) for every child in the region. Our Children’s Health℠ Pediatric Group network has been officially recognized as a PCMH by the National Committee for Quality Assurance (NCQA) for its evidence-based, patient-centered processes.

A PCMH is a model of care that encompasses all five core functions of primary health care.

Children’s Health℠ Pediatric Group services include

  • Well-child exams

  • Asthma management
  • Care coordination
  • Comprehensive evaluation and treatment of pediatric illnesses

The Children’s Health℠ Pediatric Group network has quickly expanded during the past two years and now serves families in more than 16 different locations. In collaboration with other community stakeholders, Children’s Health Pediatric Group plans to continue expansion by developing additional sites in underserved areas of the community.

Our offices are open to all children and accept most forms of insurance, including Medicaid and Children’s Health Insurance Program (CHIP).

The Children’s Health℠ Pediatric Group team is in the process of redesigning its overall approach to primary care delivery. This new care model includes telemedicine and virtual office visits. It integrates more advanced medical care into the primary care setting, delivers services through schools and faith-based organizations and enhances family engagement.
