Complex care coordination helping to improve the lives of children in the Dallas Service Delivery area pediatric preventive care, emphasizing the importance of primary and complex care medical homes.
DALLAS – Children’s Health℠, the leading pediatric health care system in North Texas, today announced it has signed an agreement with the Texas Health and Human Services Commission to administer the new State of Texas Access Reform (STAR) Kids program. Beginning in the Fall of 2016, Children’s Health, under the Children’s Medical Center Health Plan (CMCHP), will operate a Medicaid managed care program specifically serving youth and children (age 20 or younger) who are enrolled in disability-related Medicaid, including Supplemental Security Income (SSI) Medicaid or are enrolled in the Medically Dependent Children Program (MDCP).
CMCHP will provide benefits, such as prescription drugs, hospital care, primary and specialty care, preventive care, therapies, personal care services, private duty nursing, and durable medical equipment and supplies. Children and youth who get additional services through MDCP will receive additional long-term services and support through STAR Kids.
“CMCHP’s main goal is to help improve health outcomes through a family-based approach to health and well-being for children with complex medical needs,” said Christopher J. Durovich, president and chief executive officer of Children’s Health. “This means we will work with families to ensure they are receiving the right care at the right time to meet the unique needs of their children,” said Durovich.
CMCHP will focus on pediatric preventive care, emphasizing the importance of primary and complex care medical homes. We will specialize in the coordination of care for our most complex medical members to ensure they receive needed services and connect them with qualified providers in a timely manner to improve their health and independent living. Each member will have their service needs assessed, which will form the basis of that member’s individual service plan.
