Weight Management (COACH): What to Expect

Your child's primary care physician will complete an examination and lab work of your child.

When LEAN Families staff receives this information. You will be contacted to complete the following by phone:

• Health history
• Readiness to change questionnaire
• Verify insurance

Once these items are successfully completed, your family will be enrolled in the next available program class.
