Nutrition Clinic

The dietitian* may provide nutrition assessment and counseling for a variety of diagnoses:

  • Obesity or inappropriate weight gain
  • Failure to gain weight/failure to thrive
  • Feeding disorders
  • Tube feedings
  • Other special nutrition needs

*You will not be seeing a physician at your nutrition clinic visit.

The following information is required to complete your appointment:

  • A written referral from the primary physician, including clinical findings and symptoms precipitating the nutrition referral (can be MD outside of Children's)
  • Pre-authorization (required by some insurance plans)
  • Patient name, date of birth, and probable diagnosis
  • Complete growth history
  • Applicable lab results (i.e. lipid profile, glucose, hemoglobin A1C, triglycerides, albumin, and thyroid tests)
