Anticoagulation Service

A full-time pharmacist oversees this hospital-wide inpatient service, with oversight from program director, Dr. Journeycake. The pharmacist tracks patients receiving anticoagulants; coordinates education and outpatient care for those discharged on anticoagulation; and assures appropriate quality assessment and improvement activities are completed. The pharmacist is also a resource for education to medical, nursing and pharmacy staff in issues related to anticoagulation.
