Description of services:

Children in foster care are seen by a pediatric nurse practitioner with the support of physicians on the medical staff, all of whom are experienced in treating victims of abuse and neglect.

Procedures performed or problems diagnosed and treated:

• Foster care placement exams
• Pre-adoption physical and developmental screenings
• Primary care medical services for children in foster care in Dallas County, including:
• Texas Health Steps Services
• Immunizations
• Case management
• Social support

Information for referral sources:

• Services are available for children in the custody of Dallas County Child Protective Services
• Appointments can be arranged by foster parents or CPS workers.

Please provide the following when calling to schedule a patient for an appointment:

• Patient name and date of birth
• Name of CPS caseworker and date and reason for CPS custody
• Possible health concerns and immunization records
